The #1 goal of At Your Service Home Health Agency nurses is to keep patients safely at home and out of the hospital. Our nurses are responsible for evaluating the patient’s health and environment, educating the patient caregiver or other family on their medical condition. Nurses help them become more independent and coordinate care between physician, patients and their families and other care team members. We have both registered nurses and licensed vocational nurses who can provide part time or full time shift care, as well as intermittent visits, depending on the patient's needs. Our registered nurses do a complete initial evaluation and also function as case managers to ensure that our patients get all the services they need and that those services are coordinated appropriately. Our goal is to improve the patient's health condition while helping the patient to stay in the security and comfort of their own home. All of our services are provided pursuant to a doctor's order.
Our physical therapists find it very rewarding as they assist people to reclaim their mobility, independence and daily routines in their own home. The advantages of receiving physical therapy at home are mastering the daily tasks which take place in the home, and to increase safety. It’s in the home setting where most accidents occur and having the PT there allows them to assess home safety, set functional goals, and work with the patient to maximize their ability, improve strength, balance, coordination, and make strides by enhancing their quality of life as their physical impairment declines and mobility increases.
All of our home health aides are certified by the State of California. Home health aides help the patient with the activities of daily living, such as bathing, grooming dressing, and eating. They assist the patient with transferring from bed to chair to standing and walking and back again. They may also help with food preparation and keeping the home environment safe.
Our highly-qualified, dedicated nurses are available on a full-time, part-time, or per-visit basis. Spirit Home Health Care RNs and LVNs work as part of a team with the client, physician, family and caregivers, with the goal of preserving client health and averting further injury and illness. At AYSHHA, nurses perform skilled observations and assessments, wound care, catheter care, foot care for diabetic patients, and medication management. They also educate clients and their caregivers on their specific condition, and help with understanding post-discharge followup instructions.
